Office Policies – Prolase Laser Clinic

Cancellation Policy:
Prolase Laser Clinic requires at least a 24 hour notice of any cancellation or rescheduling of appointments in order to avoid a cancellation fee equal to 30% of the total treatment’s cost.

In addition to the cancellation fee, the client will need to make a pre-payment in full in order to schedule the next future appointment.

Payments & Balances:
We require that full payment is made prior to the start of treatment and that any outstanding balances be paid in-full in order to receive treatment.

The cancellation fee will automatically be applied to clients that do not show-up for their scheduled appointments.

Late Policy:
Clients arriving more than 10 minutes late to their appointments may have their treatments forfeited.

As a courtesy, we send a reminder via text message to patients at least 48 hours in advance. And, as a last attempt, we make a courtesy phone call. But ultimately, it’s the responsibility of the client to make and keep appointments regardless of whether they received a courtesy reminder or not.

We realize that unforeseen events occasionally arise, but each person’s appointment represents time, resources, and equipment reserved exclusively for them.

Surcharge for Treatments with a Doctor
Surcharges will apply for select treatments that are requested to be performed by a doctor. A $2 per unit surcharge will be applied for Botox treatments and a $50 surcharge will be applied for all laser aesthetic treatments (i.e. for skin tightening, laser leg vein treatment, acne, acne scars, IPL, and nail fungus removal).

Advanced Consultations with a Doctor or Advanced Practitioner
For consultations requiring an in-depth medical evaluation or a customized plan of care, we require a $20 advanced consultation fee. If you decide to move forward with your treatment, then the $20 fee will be credited toward your balance.

Nurse Requests
We try our best to accommodate requests for your preferred nurse. However, we cannot guarantee that you will be scheduled with your requested staff member due to unforeseen events (e.g. call-out, sick days, availability, etc…).

Pre-paid Package Treatments
Package treatments are designed to provide you with the best service at a discounted value. In addition, we ensure that your packages never expire and you may use them at your convenience. Pre-paid packages are non-refundable and non-transferable.

Minors (Under 18 years old)
Clients who are under 18 and are receiving treatment must be accompanied by a parent or legal guardian.

Child Safety
For the safety of your child, we ask that no person under the age of 12 be left unattended. Children under the age of 12 may NOT accompany you in the treatment room under any circumstances.

Service Animals and Pets
For your safety and the safety of those around you, we DO NOT allow pets at Prolase. Service animals are permitted.
